- Have you knowingly been in close or proximate contact in the past 14 days with anyone who has tested positive for COVID-19 or who has or had symptoms of COVID-19?
- Have you tested positive for COVID-19 in the past 14 days?
- Have you experienced any symptoms of COVID-19 in the past 14 days?
- Please wear a mask. If you do not have one, one will be provided for you.
- On entry, please wash your hands or use hand sanitizer.
- Come by yourself if possible. If it is necessary to bring one other person, they should be at least 6 feet away from the staff and other people.
- If you are a high-risk patient please call for an appointment and we will make special accommodations for you.
